I'll say that, as someone who worked at Noah's/ENRG for 4 years, it looks like they did actually put it in the slicer with the hope that it would be done correctly. A bread knife doesn't cut like that. Sometimes if a bagel is a bit narrow (like this), and if they put it in with a bunch of others, it might have gotten pushed on top of another one so it went past the saw at this angle.
