So I get why one of them had to have water in the brain for the joke to work but no idea why the other person needs to have a cleft palette.

The gist (though I’m not gonna translate it verbatim): Two guys one with a cleft palate and one with water on the brain are trying to figure out what to do with themselves at a sports club Guy with cleft palate says let’s do this random sporting activity, maybe tennis, and guy with water on the brain says “you idiot, I’ve got water on the brain, if I hit my head I could be seriously injured.” Guy with the cleft palate says “easy enough, there’s loads we can do. This, that. What about…swimming?” Guy with water on the brain says “idiot, I’ve got water on the brain, if I dive in wrong I could hurt myself” Guy with cleft palate says “fair enough, there’s other things we could do. How about this, that, or the sauna?” Guy with water on the brain says “the sauna sounds like a good idea! Let’s do that” So they get to the sauna and there’s another guy in there (I think). They’re enjoying themselves and he says it’s not hot enough in here, let me turn it up so he turns it up.
