I've never quite figured out the "best" way to phrase that question, but I still tried to make it as easy as possible for someone with a new name. No deep questions, just "Sure! Let's get that fixed," change the name, maybe add pronouns, make a note for the coworkers. I also liked adding in how to pronounce someone's name because addressing folks properly is good manners.

Getting rid of a dead name is like Sisyphus rolling the rock. These mother effers are in the system and you cannot find them all and purge them. You think you find the source, and yet still the name occasionally surfaces. No one knows where it's coming from. It was easier to get a passport with the new name than it was to get the dead name out of the school system(s).
